Grâce à l’Evol Driver, le Driver interdit fabriqué par Sento, et les Cobra et Rider Evol Bottles, Isurugi (ou plutôt Evolto) dira adieu à Blood Stalk pour se transformer en Kamen Rider Evol Cobra Form.

En remplaçant la Cobra Evol Bottle dans son Driver par la Rabbit Evol Bottle, il devient Kamen Rider Evol Rabbit Form. En la remplaçant par la Dragon Evol Bottle, il devient Kamen Rider Evol Dragon Form.
